Select Hawai'i residents may receive a letter in the mail requesting participation in a telephone survey conducted by SMS Hawai'i on behalf of the Hawai'i Department of Health. We only want one thing: your opinion. Results from this survey are used to improve programs that we hope will lead to better health for the people of Hawai'i.
